I recently purchased Fallout 3 GOTY (taking some time to re-live my favourite game of all time, whilst desperately waiting for a Fallout 4 announcement :)) only to find all these posts about crashes and the game being effectively unplayable. Sure enough, the game would randomly hang for me and I couldn't even make it out of Vault 101. I'm running Windows 7 x64, NVidia GTX 680, Haswell i5.
I had hangs when playing as a baby, a lot of hangs/crashes when taking the G.O.A.T. and then hangs on trying to escape the vault. The visuals would freeze, and whatever sound was playing (sometimes speech) would continue to its end but the game was frozen. I tried all the fixes mentioned in the Ultimate Setup Guide and also every other post I could find. Removing GFWL, updating fallout.ini, the d3d9x.dll, installing K-Lite, associating .ogg files, etc etc. And still, I could play the game for less than 10 minutes before a crash.
After at least one evening messing around with this, I've finally fixed the problem. I hope this provides help to at least some others who had lost all hope of getting this working. The eureka moment for me was find (by searching my PC) that...
MOD Organiser has its own fallout.ini!! C:\Program Files (x86)\Mod Organiser\profiles\Default\fallout.ini
I'm amazed that more of the guides don't mention this. They all suggest that the fallout.ini to edit is in My Documents, but if you're using Mod Organiser then you need to make sure that its own fallout.ini is edited.
TLDR: When applying:
if you're using Mod Organiser, then edit
C:\Program Files (x86)\Mod Organiser\profiles\Default\fallout.ini
instead of the file in My Documents/My Games/Fallout3!
